Notice of Petition for Adjudication of Incapacity and the Appointment of
Plenary Guardians of the Estate and Person

To:  Lewis Samuel, Sr.: 

NOTICE is hereby given that a Petition has been filed with the above-titled Court requesting the Court to declare Lewis Samuel, Jr. an Incapacitated Person.  If the Court finds him to be an incapacitated person, his rights will be affected, including the right to manage money and property and to make decisions, and guardians may be appointed.

A hearing has been scheduled for January 24, 2019 at 1:30 PM in Courtroom 416, City Hall, 1400 John F. Kennedy Blvd., Philadelphia, Pennsylvania, at which time the Judge will hear evidence in reference to the Petition.  As a party in interest, you are being given this notice of the hearing.
