Notice To Claimants of Phoenix Fire And Marine Insurance Company, Ltd.

NOTICE TO CLAIMANTS OF
PHOENIX FIRE AND MARINE INSURANCE COMPANY, LTD.

Lieutenant Governor Gregory R. Francis, in his capacity as Commissioner of Insurance and Liquidator of Phoenix Fire and Marine Insurance Company, Ltd. (“Phoenix Fire”), has authorized payment to the claimants of Phoenix Fire. If your name appears below, please contact the Office of the Lieutenant Governor, Division of Banking and Insurance (“Division”), for more information about how you may obtain payments that you may be entitled to. If your name does not appear below, but you nevertheless believe that you may have a claim against Phoenix Fire, please contact the Division to obtain more information about how you can file a claim to receive any monies that you may be entitled to. All inquiries regarding the payment of claims or the filing of claims should be made at either of the following locations:
